HoloLens keeps asking for old login after performing recovery

A colleague used one of our devices and set it up with his own account and password.
When he returned it there was a login screen asking for his password. Since I couldn't access Settings to reset the device, I installed the WDRT on a PC as recommended, and the app installed the latest version of Windows Holographic and reset the device, and I went through the initial setup again using my own account (which was successful).

The problem is, yesterday when I tried to use the device it went back to asking for my colleague's old login. I did another reset with the WDRT and went through initial setup again with the new account, but today it has again gone back to asking for my colleague's old login.

It sees like the WDRT is not actually doing a factory reset even though it's allowing me to set up the device with the new account. How can I do a proper reset and wipe my colleague's account so it won't keep locking out?
