Having read the rules, "Forms" need to be filled out. Where can i recieve these Forms?

Today is December 9th 2016 4:30 p.m. Pacific Standard Time. My name is Preston Linderman I live in Chico California and would like to receive information on how to fill out the forms said in the official rules for the actiongram contest. I would also like to find out where I can provide my contact information as I assume that I have populated my entry, during the entry period. To be sure that all Communications go smoothly I would like an email thread to start, as per communication purposes.
